When Imaging Doesn’t Match Your Pain: What MRI and Scan Results Can Miss

One of the most confusing parts of living with chronic pain is hearing that your imaging “doesn’t explain” what you’re feeling.

Sometimes that happens because the scan looks relatively normal, and the patient is left wondering how symptoms can be so disruptive if the MRI is “fine.” Other times, the opposite happens: the imaging shows multiple abnormalities, and yet none of them clearly match the pain pattern or explain why treatment has not helped. In both situations, patients are often left with the same concern — if the scan does not fully fit the symptoms, what am I supposed to do with that?

The short answer is that imaging matters, but it is only one part of the clinical picture.

MRI, CT, X-ray, and other studies can be extremely helpful. They can identify fractures, significant nerve compression, joint damage, post-surgical changes, and other structural problems that absolutely influence treatment decisions. But they do not interpret themselves, and they do not tell the whole story of chronic pain on their own.

A Scan Shows Structure. It Does Not Always Explain Symptoms.

Imaging is very good at showing anatomy. It can show disc bulges, arthritis, narrowing around nerves, tendon damage, bone density changes, scar tissue, or signs of prior injury. What it cannot do by itself is determine whether a finding is actually responsible for the pain a patient is having right now.

That distinction is important.

Many adults have imaging findings that sound concerning on paper but are not necessarily the source of their symptoms. Mild disc degeneration, arthritis, and other “wear-and-tear” changes become more common with age, even in people who are not in severe pain. A report may list several abnormalities, but that does not automatically mean every one of them is clinically meaningful.

At the same time, patients can have very real pain with relatively modest imaging findings. This is especially true when pain involves nerve sensitivity, autonomic dysfunction, connective tissue instability, post-viral illness, or conditions where the problem is not captured well on a routine scan.

Why Imaging and Symptoms Sometimes Don’t Line Up

There are several reasons this mismatch happens.

1. The finding may be real, but not the main pain generator.

A lumbar MRI might show disc degeneration, but the patient’s day-to-day symptoms may be more influenced by muscle guarding, altered movement patterns, nerve sensitization, or another overlapping pain condition. The scan is not “wrong,” but it may not be the whole explanation.

2. Pain can involve more than one mechanism at once.

Someone with osteoporosis may have vertebral changes on imaging, but also ongoing muscular pain, fear of movement, or chronic postural strain. A patient with prior surgery may have structural changes on a scan but also nerve-related pain that no longer behaves like the original surgical problem.

3. Some pain conditions do not show up clearly on standard imaging.

This is one of the biggest sources of frustration for patients. Hypermobility, dysautonomia, mast cell activation, Long COVID, central sensitization, and certain nerve pain conditions may cause significant symptoms without producing dramatic MRI findings. When that happens, patients can feel as though the absence of an obvious scan abnormality is being used to question the legitimacy of their pain.

4. Imaging findings can be overemphasized simply because they are visible.

Once a scan shows something abnormal, it is natural for the conversation to center around that finding. But visible does not always mean causative. A report can easily become the loudest part of the case, even when the symptoms themselves suggest a more complicated picture.

“Normal” Imaging Does Not Mean Nothing Is Wrong

Patients often hear some version of this: “Your MRI looks okay, so there shouldn’t be a reason for this much pain.” That can be deeply discouraging, especially when the pain is affecting sleep, mobility, work, or day-to-day function.

The reality is that not all pain is structural in the way imaging is designed to capture. A person can have significant pain from nervous system sensitization, chronic instability, autonomic dysfunction, post-viral changes, or inflammatory overlap without having a scan that clearly “proves” it.

That does not mean imaging is useless. It means it has limits.

Good pain care requires knowing when a scan is highly informative and when it is only one piece of a larger puzzle.

When Imaging Is Most Helpful

Imaging is often most useful when it is interpreted in context:

  • Does the location of the abnormality match the actual symptom pattern?
  • Does the severity of the finding fit the degree of pain or loss of function?
  • Does the timeline make sense?
  • Did symptoms change after a fracture, surgery, infection, or injury in a way that makes the imaging more meaningful?
  • Are there other factors — such as hypermobility, autonomic symptoms, or persistent post-surgical pain — that might be influencing the picture as well?

These are the questions that turn imaging from a list of findings into something clinically useful.

Why Reassessment Matters Before the Next Procedure

One of the biggest risks in chronic pain care is moving too quickly from an imaging finding to a treatment plan without asking whether the two are convincingly connected.

A patient may be told that surgery, an injection, neuromodulation, or another intervention is the next logical step because the MRI shows a structural issue. Sometimes that is absolutely appropriate. But when the pain pattern, physical function, and prior treatment response do not clearly support that recommendation, it is worth slowing down.

This is where a second opinion can be particularly valuable. Not because the scan needs to be “re-read” in isolation, but because the broader question needs to be asked: does this imaging actually explain what is happening well enough to justify the next intervention?

The Goal Is Not More Doubt. It’s Better Interpretation.

Patients do not need to become radiologists. They do not need to memorize every line of an MRI report or worry that every abnormality is dangerous. What they do need is a clear understanding of what matters, what may be incidental, and what still does not add up.

For many patients, the most important question is not whether the scan shows something. It is whether that something truly explains the pain well enough to guide the next decision with confidence.

When imaging and symptoms do not seem to match, that does not always mean something has been missed. But it often means the case deserves a more careful interpretation before moving forward.
